Comprehensive and effective discharge planning is imperative to ensure continuity of care to support improved patient/resident health outcomes and safety at transfer and discharge to the community. This webinar program will provide an overview of the 6/6/23 Centers for Medicare & Medicaid Services (CMS) Quality, Safety & Oversight (QSO) 23-16 – Hospital Memorandum of Requirements for Hospital Discharges to Post-Acute Care Providers (https://www.cms.gov/files/document/qso-23-16-hospitals.pdf) and focus on areas identified by CMS as opportunities for improvement related to inclusion and accuracy of critical patient information. The CMS Conditions of Participation specific to the discharge planning process will also be reviewed as well as resources to facilitate accurate and timely information exchange with providers across the healthcare continuum.
