CDC NHSN Special Circumstances

The Centers for Disease Control and Prevention’s National Healthcare Safety Network (NHSN) continues to support the nation’s COVID-19 response for Long-Term Care Facilities. The NHSN Long-term Care Facility (LTCF) COVID-19 Module webpage has many resources outlining facility enrollment steps and live Q&A opportunities.

However here are some specific instructions for unique situations:

Your NHSN facility administrator has not accessed NHSN in over a year and your facility is enrolled in NHSN, then your access needs to be reinstated. Please:

Identify someone in your facility to be added as a facility admin, then contact us at nhsnenrollment@cdc.gov with subject line May 12 LTCF Support to request SAMS access for COVID-19. This will allow you to report your COVID-19 data.

After your data has been entered, please then send an email to the nhsn@cdc.gov email address for SAMS access for all NHSN reporting modules, including UTI.

Your previously assigned NHSN facility administrator is no longer at the facility and your new facility administrator requires access to NHSH, then contact nhsnenrollment@cdc.gov with subject line May 12 LTCF Support to be added as a user requesting SAMS access for COVID-19  to report your COVID-19 data.

After your data has been entered, please then send an email to the nhsn@cdc.gov email address for SAMS access for all NHSN reporting modules, including UTI.

You’re unsure if your facility is enrolled in NHSN, then contact us at nhsnenrollment@cdc.gov with subject line May 12 LTCF Support.
