The IPRO-QIO Community Resource Tool was developed to facilitate discharge planning and improve care transitions.
The services and supports listed below supplement existing resources and are periodically updated with user notifications.
- Office of the Aging Offices (OFA): Organizations providing seniors with information, support, and services, including home-delivered meals, senior center programming, health promotion, care coordination, and more.
- Community Action Agencies (CAA): Local agencies empowering individuals at risk or in poverty through services like food and nutrition assistance, job and career support, housing, and family assistance.
- Independent Living Centers (ILCs): Community-based organizations offering support to individuals with disabilities, including advocacy, independent living skills, and access to services.
- Salvation Army: Provides various services to those in need, such as homeless shelters, food pantries, emergency assistance, and rehabilitation programs.
- Catholic Charities: Offers services based on community needs, including emergency assistance, housing, counseling, and immigration support.
- Department of Social Services (DSS): Provides financial assistance, food stamps, healthcare programs, child welfare services, employment support, housing assistance, and homeless prevention services.
- Emergency Shelters: Facilities offering refuge and necessities to individuals and families affected by emergencies or disasters.
- Federally Qualified Health Clinics (FQHC): Community-based healthcare organizations providing primary care, medical, behavioral health, and dental services to underserved populations on a sliding fee scale.
- Food Pantry: Community-based organizations providing free food to individuals and families in need to address food insecurity.
- Nursing Home (NH) / Skilled Nursing Facilities (SNFs): Residential care facilities offering 24/7 medical and personal care to individuals with chronic medical conditions or in recovery.
- Certified Home Health Agency (CHHA): Healthcare provider offering skilled nursing services and support for activities of daily living in the home setting.
- Inpatient Rehab Facility (IRF): Healthcare facility providing intensive treatment and care for medical conditions, injuries, or substance abuse problems with 24/7 patient stays.
- Substance Abuse Disorder Treatment (SUD): Interventions, therapies, and support services for individuals struggling with substance abuse or addiction. There are two other sub-categories as well, SUD-DETOX-OUD, or Substance Abuse Disorder treatment services, including detox and Opioid Use Disorder, and SUD-OUD, Substance Abuse Disorder treatment including Opioid Use Disorder.
- Long-Term Acute Care Hospital (LTACH): Specialized healthcare facility providing extended medical care for patients with complex conditions.
- Mental Health (MH): Wide range of resources and interventions for addressing mental health and emotional well-being, including therapy, counseling, and residential treatment.
- Assisted Living: Residential care facility providing support and assistance to seniors or individuals with disabilities for daily activities.
- Transportation: Programs offering accessible and affordable transportation options for individuals with mobility or financial challenges.
- Dialysis Facilities Centers (DFC): Medical centers providing dialysis treatment for patients with kidney failure or end-stage renal disease (ESRD)
