Survey Readiness & Documentation

This section helps providers organize Appendix Z documentation, reduce common compliance gaps, and demonstrate that emergency preparedness activities are implemented—not just written. Survey readiness should also strengthen operational capability by showing that plans, policies, training, exercises, and improvement actions work together.

Appendix Z Evidence Binder Checklist

Current facility-based and community-based risk assessment / HVA
Emergency preparedness plan and documented review/update evidence
Policies and procedures that support response and continuity of care
Emergency communication plan, contact lists, and redundant communication evidence
Training records and annual/ongoing staff readiness documentation
Exercise, drill, tabletop, or full-scale exercise documentation
After-action reports and improvement plan tracker
Partner coordination evidence such as healthcare coalition, emergency management, public health, or MOU documentation where applicable
Evidence that corrective actions have been assigned, tracked, and incorporated into plan updates
