Changes to Weekly NHSN COVID-19 Vaccination Data Collection Forms

Additional/Secondary Vaccination Series:
First Update August 2021

What are the changes?

In anticipation of recommendations for additional vaccine series, new questions on vaccine doses will be added to the data collection forms. The main changes are outlined below.

  1. Question #2 remains the same, except it asks about the primary vaccination series.
  2. Added question #4 for facilities to report on the cumulative number of individuals eligible to receive an additional dose of COVID-19 vaccine.
  3. Added question #5 for facilities to report on the cumulative number of individuals who received an additional dose of COVID-19 vaccine (by manufacturer type).

Second Update Coming this Fall

Don’t forget: Healthcare personnel categories for NHSN COVID-19 vaccination data collection forms will be revised.

As noted last month, the training webinars on additional changes to the NHSN application, such as the new healthcare personnel categories, will occur later this Fall.

Will there be training webinars?

Yes! CDC will be conducting training webinars in late August. The goal is to focus on one update at a time. The first training will provide an update about the additional dose questions shown on the sample form above.
