The goal is to enhance community health and focus on CMS quality goals: reduce opioid misuse, increase prevention and management of chronic conditions, enhance care coordination, further immunizations, support emergency preparedness and improve infection control practices by:

• Taking a data-driven approach to explore critical quality opportunities.
• Learning new evidence based interventions and promising practices.
• Engaging in peer-to-peer sharing (which comes from you) and problem solving together.

Join in the quarterly session that works best for you and if you are unable to attend, consider asking a colleague to participate on your behalf.
