A care transition is the movement of a patient from one health care provider or care setting to another. A transition can be complex and a patient can have an increased risk of infection, adverse drug event (ADE), or miscommunication.

Cross-setting collaboration and partnership with other health care and community-based providers are essential strategies to reducing avoidable admissions, readmissions, infection, ADE, while also improving communication.
