COVID-19 Workshop on Jan. 14: Innovative Home Care Collaboratives

Thursday, January 14, 10:30-11:00am

Join the IPRO QIN-QIO as Al Cardillo, President and CEO of the Home Care Association of New York State, provides information about innovative collaborations between home care and hospitals. Collaborations include ways to address COVID-19 and chronic care management issues during transitions of care.

We’ll also present other unique home care collaborations, such as a home care project with county health departments to provide in-home specimen collection for COVID-19 testing.

Join us on January 14 and discover:

  • A concept of hospital-home care synchronization for front-end/pre-acute hospital care, far-end/post-hospital care, recovery, and long term support, called “New York Statewide Hospital-Home Care Collaborative for COVID-19 and Beyond.”
  • Prototypes of hospital and home care collaboration models that can be emulated by other providers — working together across settings.
  • Home care training opportunities in New York State for in-home specimen collection for COVID-19 testing and county health department collaboration with home care for in-home specimen collection for COVID-19 testing.
