In this series, we will review factors that can contribute to readmissions and strategies to address these factors. Some topics to be discussed are C.Diff, Sepsis, UTIs, Dementia, Falls, Wound Care, and CHF.
The series will guide participants through the world of nursing home quality improvement – the process, the tools, exercises to transform your practice, ways to engage your team in QI, case studies and valuable learning taught by our team of quality improvement specialists. As we move deeper into the program, we will target specific disease processes, and offer best practices to reduce the potential for emergency department visits and/or readmissions.
The series will run Tuesdays at noon ET beginning October 18, 2022. Each week there will be a live replay on Wednesdays at 3:30 pm ET. As always, join us on Thursdays at 11 am ET for IP3 Office Hours.
