Join us on February 16 at 12pm ET to learn about NYC Health + Hospital | Queens’ experience and outcomes achieved through participation in the NYS Department of Health Medicaid Accelerated eXchange (MAX) Series focused on improving care coordination for the super utilizer population. This program supports the goal of transforming the system by strengthening care collaborations and getting patients the right care at the right time. By understanding the drivers of high utilization, patients are directed to appropriate community providers and resources while promoting the more efficient use of hospitals for emergent and acute level services.
