Dr. Edward Chapman, Healthcare Provider

Dr. Edward Chapman is a physician specializing in internal medicine and addiction medicine. He has practiced in Washington, DC for more than four decades. His research interests include addiction, mental illness, infectious diseases (AIDS and hepatitis C), criminal behavior, and chronic diseases. A Howard University College of Medicine graduate, he completed his internship and residency in internal medicine and fellowship in cardiology at Freedmen’s and Howard University Hospitals. He is board certified in internal medicine and addiction medicine, an American Society of Addiction Medicine Fellow, and an Alpha Omega Alpha Honor Medical Society Member.
Dr. Samaria R. Washington, Regional Director of SNF Operations, Bridge Point Healthcare

Dr. Samaria R. Washington is a licensed nursing home administrator. She has over 25 years of experience and specializes in skilled nursing and rehabilitation, assisted living, policy analysis, staff education, and governmental regulations. She has developed growth and expansion projects to include ventilator units, dialysis services, and person-centered care facility improvements. She is the current District of Columbia Health Care Association Immediate Past Chair and a LifeSpan Network Board Member. In 2023, she was a contributing author of The Doctor Talker.
Tina Sandri, President, Forest Hills of DC

Tina Sandri is President of Forest Hills of DC, a 135-year-old senior living services community, including assisted living, skilled nursing, and memory care. A second generation healthcare leader, she has led in for-profit, non-profit, chain, and free-standing healthcare environments, including hospitals and continuing care retirement communities. She serves on the American Health Care Association Board of Governors and has served on the LeadingAge DC Board. She also serves on the Moving Forward Steering Committee and the Vision Centre Advisory Council. She has been a Baldrige Gold Quality Award Examiner and an AIT Preceptor.

Leslie Graham, Immediate Past President and Chief Executive Officer, Primary Care Coalition

Leslie Graham, Immediate Past President and Chief Executive Officer of the Primary Care Coalition, has dedicated her career to improving the health of populations at local, state, and international levels utilizing the principles of quality improvement. Career achievements include co-founding a partnership among six hospitals, local nonprofits, and the local government that reduced readmissions from skilled nursing facilities to hospitals, reduced emergency department use among behavioral health high utilizers, and supported hospitals in addressing patient social needs. She has an Master’s in Healthcare Administration, and a Bachelor of Science in Computer Science. In 2023, she was named one of Maryland’s Top 100 Women.

Bonnie Horvath, Patient Representative

Ms. Horvath joined Qlarant in 2006. She is the former Program Director for the Medicare Quality Improvement Organizations for the District of Columbia, Delaware, and Maryland as a part of the IPRO QIN-QIO. In previous roles, she has designed, executed, and operated multiple collaboratives including the Maryland Patient Safety Center Perinatal, Neonatal, and Hand Hygiene Collaboratives and the national Centers for Disease Control and Prevention Hemodialysis Blood Stream Infection Prevention Collaborative. She was also a part of the National Content Developer Team for the Partnership for Patients, a Centers for Medicare & Medicaid Services hospital patient safety initiative.

Chris Fisher has served as the Director of Quality and Clinical Affairs at the Pennsylvania Healthcare Association (PHCA) since January 2018. He works with providers to improve quality within their organizations through clinical, operational, and process improvements. He monitors, informs, and educates providers on most recent Centers for Medicare & Medicaid Services updates and state initiatives in Quality Measures and performance. He also serves as a Senior Level Quality Award Examiner and a Team Leader through the American Health Care Association/National Center for Assisted Living.

Hallie Morgan, Vice President, Quality and Data Services, West Virginia Hospital Association

Hallie Morgan serves as the Vice President of Quality and Data Services with the West Virginia Hospital Association (WVHA). In her current role, she is responsible for the planning and development of quality activities and leads the association’s analytic programs. She provides guidance on regulatory and legislative issues and assists with advocacy in support of the association’s strategic initiatives, including quality, access to care, and workforce development. She joined WVHA in 2015 as the Director of Data Analysis where she was integral in developing key WVHA reporting programs. She holds a Master of Public Health degree from the University at Albany, a Master of Arts degree in community counseling from West Virginia University, and a bachelor’s degree in psychology.

Amber Furman, a lifelong resident of West Virginia, brings 25 years of medical expertise and a strong leadership background to her role as Director of Workforce Development at the West Virginia Hospital Association (WVHA), a position she has held since 2023. She leads efforts on workforce retention and development, connecting stakeholders, supporting key programs, and leveraging data-driven insights for strategic decision-making. Before joining WVHA, she spent 23 years at Radiology, Inc., where she managed finances and staff, demonstrating her ability to lead and optimize operations. She holds a Master of Healthcare Administration and a Bachelor of Business Administration from Marshall University.
