Region 2 Electronic Provider Service Agreement

The Centers for Medicare & Medicaid Services (CMS) Quality Innovation Network–Quality Improvement Organization (QIN-QIO) program supports hospitals, nursing homes, and outpatient clinical practices in advancing the health, safety, and outcomes of Medicare beneficiaries.

Our team of quality improvement advisors will partner with your organization to strengthen your quality management infrastructure and support your improvement prioritiesand data collection processes. Focus areas and strategies will be reviewed and refined over time to ensure alignment with your goals and the evolving needs of your patients and community.

We will support your organization by:

  • Assessing quality and technology needs
  • Developing a Quality Action Plan
  • Providing tailored technical assistance
  • Supporting evidence-based interventions
  • Using quarterly data for benchmarking and performance tracking
  • Building peer learning and collaboration connections
  • Sharing your feedback with CMS to enhance the CMS’ QIO program and our support

Our quality improvement advisors will help you select and apply evidence-based interventions to improve the quality of care, safety, and outcomes for the people you serve in one or more of the following focus areas:

  • Prevention & Management: Type 2 Diabetes, Hypertension, Vaccinations
  • Behavioral Health:  Chronic Kidney Disease, Depression and Suicide Prevention, Substance Use Disorders, Chronic Pain
  • Patient Safety: Infection Prevention and Control, Adverse Drug Events, Safety Events
  • Care Coordination: Readmissions to Hospitals from SNF, Emergency Department Utilization, Hospital 30-Day Readmissions
  • Quality Management Infrastructure: Workforce Planning, Emergency Preparedness, Advancing Healthcare Quality Through Technology

Participation in this program is voluntary and extends through May 2030. This agreement may be terminated at any point during the QIN-QIO program due to any of the following: breach of any part of the agreement, loss of Medicare certification, permanent closure of the provider, documented refusal of the provider to participate, or as decided by CMS.

Confidentiality & Conflict of Interest Statement

This Participation Agreement shall establish a “covered-entity” relationship between the QIN-QIO and the healthcare provider facility or outpatient clinical practice being supported. In accordance with federal statutes and regulations in Part B of Title XI of the Social Security Act and 42 CFR Part 480, protected health information can be disclosed to a QIN-QIO. Under contract with CMS, QIN-QIOs serve as health oversight agents to CMS and as such, we are subject to applicable privacy and security requirements of the Health Insurance Portability and Accountability Act (HIPAA), the Health Information Technology for Economic and Clinical Health (HITECH) Act and implementing regulations at 45 CFR Parts 160 and 164. Under this Participation Agreement, your organization confirms there are no conflicts of interest with the Mid-Atlantic CMS QIN-QIO (Region 2) or any other federal agency, and both entities agree to comply with CMS Privacy and Security Requirements and all applicable laws and regulations for the storing, maintaining, and transfer of health information.
