DESCRIPTION:Session 1 – December 8\, 2022 \nJoin IPRO for the Health Equity Organizational Assessment (HEOA) Affinity Group. Led by IPRO Health Equity subject matter experts (SMEs)\, the group will work together to advance opportunities for improvement identified from your organization’s HEOA report. The affinity group will give participants the opportunity to collaborate\, share best practices and lessons learned while implementing the key activities in the HEOA. \nAffinity Group Objectives \n\nEngage in a hands-on\, interactive learning experience where participants take action and have demonstrable achievements to share within their organizations and among peers.\nAssist participants to select and implement key activities for at least one HEOA category where opportunities for improvement exist.\nProvide a collaborative forum for participants to share best practices\, challenges\, and lessons learned from implementing the HEOA key activities.\nShare tools\, resources\, and other health equity related material among affinity group participants.\n\nIPRO will sponsor a private Facebook group for affinity group participants to collaborate and receive support between sessions. The IPRO facilitators will monitor the Facebook page to ensure that questions and requests for support are addressed in a timely manner\, and share relevant resources. \nAffinity Group Guidelines \nWe are excited you are interested in the HEOA Affinity Group to advance health equity. This will be a highly interactive\, hands-on learning experience where participants will implement a small-scale quality improvement project related to their organization’s HEOA. IPRO Health Equity SMEs will facilitate this group and offer technical assistance as needed. DESCRIPTION:In the US an estimated 12\,800 people died from Clostridioides difficile (C. difficile) in 2017\, and the CDC Threat Report lists the infection as an urgent health issue. Unfortunately\, C. difficile is more common and severe in older people. Inappropriate antibiotic prescribing puts patients at risk for C. diff infections. DESCRIPTION:Speaker: Ghinwa Dumyati\, MD\, Director Communicable Diseases Surveillance and Prevention Program\, University of Rochester Medical Center. \nDr. Dumyati has expertise in the prevention of healthcare associated infections and antibiotic stewardship. She collaborates with the CDC Emerging Infection Program on identifying the burden and risk factors of infectious diseases of public health importance such as C. difficile and antibiotic resistant gram negative organisms. Dr. Dumyati led several collaborative quality improvement projects to reduce the burden of healthcare associated infections. Over the past several years she succeeded in reducing the incidence of C difficile infections in the local hospitals and nursing homes. Dr. Dumyati is a member of the Society for Post-Acute and Long-Term Care Medicine (AMDA) and is the Chair of its Infection Advisory committee. She is also a Fellow of the Society for Healthcare Epidemiology of America and is a member of the Quality Measures Task Force as well as member of its long term care interest group. \nAt the conclusion of this program\, participants will be able to: \n\nList the most recent treatment guidelines for C. difficile infection\nIdentify the role of diagnostic stewardship in optimizing C. difficile testing\nDescribe the importance of communication across care transitions in improving the management of patients with C. difficile infection
