SUMMARY:TAP: A Strategy to Prioritize and Implement HAI Prevention Efforts in Your Hospital
DESCRIPTION:Join us for a webinar featuring subject matter experts from the Centers for Disease Control and Prevention (CDC) to learn about a comprehensive strategy to reduce hospital acquired infections (HAI). The Targeted Assessment for Prevention (TAP) Strategy is a CDC-developed framework for quality improvement that uses data to drive interventions that will prevent healthcare-associated infections. \nThe TAP strategy targets healthcare facilities and specific units within facilities that have a disproportionate burden of HAIs so that prevention efforts can be prioritized and implemented where they will have the greatest impact. Our speakers will discuss the latest updates in the strategy and tools\, including adaptation for smaller facilities. \nYour participation in this webinar will provide you with updated information on: \n\nHow to use NHSN collected data to identify opportunities for improvement by facility and unit.\nHow to calculate a cumulative attributable difference (CAD)* to identify a concrete infection reduction goal\nHow to use CDC assessments to target the focus of improvement work and identify high value interventions.\n\n*The number of infections that must be prevented to achieve an HAI reduction goal.  \nWho should attend: \n\nClinical staff\nInfection control practitioners\nPharmacists\nQuality improvement staff\n\nSpeakers:\nKatie White\, MPH\, CIC \nEpidemiologist Division of Healthcare Quality | Promotion Prevention and Response Branch \nCenters for Disease Contol and Prevention \nRonda Cochran\, MPH    \nDeputy Team Lead Hospital Infection Prevention Team \nPrevention and Response Branch Division of Healthcare Quality Promotion (DHQP) \nCenters for Disease Control and Prevention \nRachel Snyder\, MPH  \nScientific Data Analyst Chenega Enterprise Systems and Solutions (chESS) \nPrevention and Response Branch Division of Healthcare Quality Promotion (DHQP) \nCenters for Disease Control and Prevention \n  \nWatch the Recording\nView the Slides

SUMMARY:IPRO HQIC Sepsis Affinity Group: Mid-to-large sized Hospitals
DESCRIPTION:Sepsis remains a national priority and is a complex and challenging condition to manage. Despite substantial attention to improvement\, an opportunity still exists to decrease sepsis-related morbidity and mortality. More than 1.7 million adults in the U.S develop sepsis every year\, leading to over 270\,000 deaths (CDC\, 2020). \nHospitals are committed to improving care\, outcomes\, and costs; however\, many have expressed they wrestle with where and how to start their quality improvement efforts. \nIPRO is launching a 6-month Sepsis Affinity Group to help support and provide a networking forum specifically for mid-to-large sized hospitals as they work to reduce sepsis-related harm. With the shared goal of accelerating learning and improvement\, participants will have access to content experts and a platform to share experiences\, evidence-based practices\, and tools\, as well as explore successes\, challenges and lessons learned to drive progress on sepsis-related harm. Some topics to be included are sepsis screening\, SEP-1 bundle compliance and reporting\, and more. \n  \nOn behalf of the IPRO HQIC team\, we look forward to your joining and working together with the Sepsis Affinity Group! \n  \nView Slides\n

SUMMARY:Partnering Beyond COVID-19: Learning Circle: Diabetes/Kidney Health
DESCRIPTION:Session Topic: Learning Circle: Diabetes/Kidney Health  \nDescription: Review and Check- in  \n  \nCOVID-19 has changed healthcare in many ways and has delayed chronic disease management for many patients. Patients needing follow-up appointments for their chronic disease were not able to schedule appointments due to decrease in in-person visits\, laboratory closures for routine testing\, and less access to community-based programs.  \nOur program over the next few months will share evidence based practices and innovative strategies that have emerged to improve prevention and management of chronic disease and outcomes.  \n  \nUpon completion of this educational series\, participants will be able to:  \n\nParticipants will develop deeper understanding of best practices and evidence-based tools for prevention and management of hypertension\, chronic kidney disease (CKD)\, and diabetes\, \nParticipants will enhance their knowledge of Million Hearts\, TAKEHeart and IPRO QIN-QIO tools for increasing referrals to and participation in cardiac rehabilitation programs\, and \nParticipants will enhance their knowledge about chronic disease health disparities and the best practices for addressing these disparities\, to support better health outcomes.

SUMMARY:Transitions in Care: Preventing Sepsis-Related Readmissions
DESCRIPTION:Anyone can develop sepsis\, a life-threatening\, complex and challenging condition to manage. There are significant human and financial costs associated with it. Every year more than 1.7 million adults in the U.S. develop sepsis\, leading to over 270\,000 deaths. Roughly 19% of sepsis survivors are readmitted to the hospital within 30 days\, leading to $3.1 billion in annual costs. Well-defined systems and processes across the continuum are crucial to delivering the right care at the right time to the right patient. \nThis event will feature proactive transitions in care and hand-off strategies to the next level of care provider to improve patient outcomes and prevent sepsis-related readmissions. The patient voice will be highlighted via a sepsis survivor story. Key discharge planning\, patient and family engagement\, health equity\, infection prevention and patient education tactics will also be shared. \nLearning Objectives:\nThrough this program\, participants will: \n\nReview successful care coordination and partnership strategies across the care continuum to prevent sepsis-related hospital readmissions.\nExplore promising practices for overcoming challenges that affect handoffs to the next level of care.\nLearn how to engage patients and families as partners and integrate their health-related social needs into the discharge planning process to improve patient outcomes.\nHear about a sepsis survivor story\n\n  \nWho Should Attend:\nNurses\, Sepsis Coordinators\, Care Coordinators\, Front-line and Emergency Department Staff\, Infection Preventionists\, Clinical and Hospital Leaders\, Physicians\, Patient Safety and Quality Professionals. \n  \nView Slides\nWatch the Recording\nChange Pathway
