DESCRIPTION:An innovative program addressing patient and family satisfaction\, readmission reduction\, and patient-centered care goals \nThe Eastern US Quality Improvement Collaborative\, a CMS Task  III  contractor in Connecticut\, New Hampshire\, New York\, North Carolina\, Vermont\, and West Virginia\, invites SNF\, home care\, and community agencies to learn about its innovative care partner model. \nThe next session in our Care Partner sprint is dedicated to the role of community-based organizations\, and we hope it will be very beneficial to the hospitals\, SNFs\, home care\, and other community hospitals. \nA quality director from one of our EQIC hospitals will discuss their Care Transition Community Collaborative developed with the assistance of IPRO’s QIN-QIO\, which includes the identification and involvement of community-based organizations. \nAs the new care partner model grows over the next few years\, we believe that more and more patients and families will be using and referring to their care partner. We want to ensure that our partner organizations understand the model as it becomes more prevalent patient care. The attendance of hospitals outside of EQIC is an important collaborative step for the Task I and Task III participants to work together. \n

He has participated in developing and co-authoring opioid-related practice and consensus guidelines. \nHe is owner and managing editor for paindr.com and Founder and CEO of Remitigate LLC\, a software development company that has launched an application to help clinicians interpret urine drugs screens and another that interprets risk of opioid-induced respiratory depression and guides clinicians in qualifying patients for in-home naloxone. Dr. Fudin is a prolific lecturer\, writer\, editor\, teacher\, and researcher\, with over 350 publications on pain management topics. \n \nThe Connecticut Pharmacists Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Administrator Brooks-LaSure is the first Black woman to serve as Administrator of CMS and has made health equity a top priority of the agency which provides health coverage to 150 million people nationwide through Medicare\, Medicaid & CHIP and the Marketplaces. \nIn this roundtable\, we’ll explore themes of the work CMS can do to further address racial disparities and advance health equity. We’ll also discuss how Black leaders are making history in the health care system and the unique role HBCUs play in rebuilding the healthcare system\, particularly in a post-COVID world. DESCRIPTION:This event is hosted by HealthBegins \nHealthcare leaders and practitioners around the country are increasingly understanding the urgency of two foundational goals—addressing health-related social needs and improving racial equity—and seeking ways to act on them. Yet there’s an important truth that must be applied for these efforts to succeed. It is that meeting social needs and improving racial equity are not separate pursuits. Without a focus on equity within health-related social needs interventions\, underlying inequities can persist. To be effective\, the two must go hand in hand. \nPioneering institutions across the country are already answering the call to intentionally center racial equity in social needs interventions. In this webinar\, we will hear directly from some of them as they share lessons learned from the field. These panelists\, all participants in HealthBegins’ training and education programs\, work in settings from health systems to CBO’s. DESCRIPTION:Blue Cross Blue Shield of Massachusetts (Blue Cross) shared the results of a new analysis of racial and ethnic inequities in health care and became the first health plan in Massachusetts to announce it will incorporate equity measures — differences in the quality of care across racial and ethnic groups — into its contracts and payment programs with clinicians who care for Blue Cross members. In preparation for equity-based payment incentives\, BCBSMA has partnered with the Institute for Healthcare Improvement (IHI) to convene an Equity Action Community to develop equity-improving capabilities among providers and facilitate collaboration among them. \nNCQA will host Dr. Mark Friedberg\, Senior Vice President\, Performance Measurement & Improvement at Blue Cross and Shannon Welch\, MPH\, Senior Director at Institute for Healthcare Improvement\, for a presentation on this groundbreaking work. Join faculty for an overview of Blue Cross’ plan to address health inequity and racial injustice\, results from the recently released Health Equity Report and the launch of a new collaborative intended to assist physicians and hospitals in improving racial inequities in care.
