DESCRIPTION:Join the New England Rural Health Association and the Northeast/Mid-Atlantic CMS QIN-QIO on Thursday\, August 20 at 11 am ET for a 45-minute executive panel discussion on the Rural Health Transformation (RHT) initiative. \nHear an overview of RHT\, get a closer look at New Hampshire’s GO NORTH* state plan\, and learn how QIN-QIO support can help rural healthcare providers turn new investments into meaningful\, sustainable improvement. Using remote patient monitoring as one example\, panelists will show how QIN-QIO support can complement state-funded resources with patient identification\, workflow design\, staff training\, patient-facing tools\, and data to measure impact. \nLearn how to maximize RHT opportunities and accelerate implementation across rural health priorities. \n \n* In New Hampshire\, The Governor’s Office of New Opportunities and Rural Transformational Health (GO-NORTH) was established by Executive Order under Governor Kelly Ayotte to lead implementation of New Hampshire’s Rural Health Transformation Program. GO-NORTH is implementing the program through five Hub contracts\, of which the Foundation for Healthy Communities (FHC) is one.
