He is a principal of TMIT Consulting\, LLC and a care transformation subject matter expert helping VA become a learning health system and achieve the quintuple aim. He is the lead author on influential guidebooks and national roadmaps for achieving these goals and brings together diverse stakeholders to implement the recommendations (e.g.\, via initiatives such as POLLC and ACT) and accelerate improvements in care delivery and results. \nAnne Myrka\, RPh\, MAT\, CPHQ – Senior Director of Drug Safety ad Chronic Disease Management\, IPRO\nAnne Myrka earned a Bachelor of Science in Pharmacy from Albany College of Pharmacy and Health Sciences and a Master of Arts in Teaching Science from the State University at New Paltz. She is a licensed pharmacist in New York and Vermont. She has presented nationally about high-risk medication management\, care transitions\, and medication reconciliation\, and has co-authored multiple peer-reviewed journal articles on these topics. \nLynn Wilson\, MS – Senior Quality Improvement Specialist\, IPRO\nLynn Wilson has more than 30 years of experience with regulatory agencies for community mental health centers and nursing homes and is a nationally recognized QI leader in long-term care\, behavioral health and hospice and palliative care settings. Under her leadership\, behavioral health and end of-life care practice innovations generated through frontline staff quality improvement processes have been recognized as national best practices.
