SUMMARY:Voices from the Field Series: Integrating Z-Codes into Your Social Determinants of Health (SDOH) Strategy
DESCRIPTION:Hospitals and health systems are working to address the non-medical factors that influence health\, known as social determinants of health (SDOH). Although not yet widely adopted\, Z-codes are important to your strategy. In this session\, you will hear how a hospital—through a pilot with their EHR vendor—advanced their SDOH strategy while embracing their organizational\nculture. \nAdditionally\, you will learn from a subject matter expert about the nuts and bolts of integrating Z-codes into your collection and representation of performance and financial metrics. \nIn this session\, you will learn: \n\nHow the Valley Health System in New Jersey developed a pilot program with their EMR vendor to screen for SDOH and integrate select Z-codes into a bill-based structure. They did this by conducting internal and external scans of their patients served and available community resources to address targeted social determinants of health needs.\nHow collecting and reporting Z-codes will impact financial reimbursement by linking social determinants of health\, patient acuity\, and other risk factors that will be important to ensure reimbursement and performance metrics which will accurately reflect the breadth and depth of patient need\, services delivered\, and actual health outcomes.\n\nWho Should Attend? \n\nPopulation Health Managers\, Case Managers\, Discharge Planners\, Quality Improvement Teams\, Social Workers\, Admissions\, Community-Based Organizations\, Post-Acute Care Providers\, Nurses\, Hospitalists\, NPs/PAs\, Physician Practices\n\nThis program will be recorded and available for access post-event \nIf you have any questions\, please reach out to your Senior Quality Improvement Specialist: \n  \nCindy Stark- cstark@ipro.org– NY and NJ \nFred Ratto- fratto2@ipro.org– NY and NJ \nGail Gresko- ggresko@ipro.org – OH

SUMMARY:Partnering Beyond COVID-19: Management of Patients with Chronic Conditions: Kidney Health: CKD
DESCRIPTION:Session Topic: Management of Patients with Chronic Conditions: Kidney Health: CKD  \nDescription:  \nCKD affects more than 1 in 7 U.S. adults.  For patients with diabetes or high blood pressure\, the risk for CKD is even greater.   \nInterventions that can slow progression of CKD include early identification\, controlling blood pressure\, controlling blood glucose\, reducing albuminuria\, eating a healthy diet and maintaining a healthy lifestyle.   \n  \nJoin us to learn more about early identification of CKD\, treatment and monitoring progression\, and delivering patient-centered care.   \n

SUMMARY:Nursing Home Sepsis Learning Circle
DESCRIPTION:Sepsis impacts individuals across the continuum of care\, with 80%-90% of sepsis-related infections occurring in the home and community. Every two minutes\, someone dies of sepsis\, leading to more than a quarter of a million sepsis deaths each year in patients both young and old. Every 20 seconds a person is hospitalized with sepsis\, and one-in-four are re-hospitalized within 30 days. Sepsis deaths can be substantially preventable\, but the solution requires participation and collaboration across the entire health care system. The Sepsis Learning Circle offers a forum for education and sharing of resources and best practices targeting early identification of sepsis with a goal for all participants to benefit from the wisdom and experience of the group to improve resident outcomes. \n  \nJoin other clinicians and subject matter experts in conversation focused on sepsis identification and treatment as a medical emergency. \nIPRO QIN-QIO Sepsis Learning Circle meetings will provide the opportunity to: \n\nDiscuss the signs and symptoms of sepsis\, the importance of early recognition and established protocols for emergent treatment.\nAsk a subject matter expert about sepsis protocols in the long-term care setting.\nReview the components of a turnkey Sepsis Train-the-Trainer Toolkit with materials and talking points to train all levels of staff within your facility.\n\nSubject Matter Expert: \nSara Butterfield\, RN\, BSN\, CPHQ \nAssistant Vice President\, Healthcare Quality Improvement Program \n  \nPlease note: Session Two takes place on Wednesday\, June 28 from 12 – 1 pm. Register here. \n  \n

SUMMARY:TAP: A Strategy to Prioritize and Implement HAI Prevention Efforts in Your Hospital
DESCRIPTION:Join us for a webinar featuring subject matter experts from the Centers for Disease Control and Prevention (CDC) to learn about a comprehensive strategy to reduce hospital acquired infections (HAI). The Targeted Assessment for Prevention (TAP) Strategy is a CDC-developed framework for quality improvement that uses data to drive interventions that will prevent healthcare-associated infections. \nThe TAP strategy targets healthcare facilities and specific units within facilities that have a disproportionate burden of HAIs so that prevention efforts can be prioritized and implemented where they will have the greatest impact. Our speakers will discuss the latest updates in the strategy and tools\, including adaptation for smaller facilities. \nYour participation in this webinar will provide you with updated information on: \n\nHow to use NHSN collected data to identify opportunities for improvement by facility and unit.\nHow to calculate a cumulative attributable difference (CAD)* to identify a concrete infection reduction goal\nHow to use CDC assessments to target the focus of improvement work and identify high value interventions.\n\n*The number of infections that must be prevented to achieve an HAI reduction goal.  \nWho should attend: \n\nClinical staff\nInfection control practitioners\nPharmacists\nQuality improvement staff\n\nSpeakers:\nKatie White\, MPH\, CIC \nEpidemiologist Division of Healthcare Quality | Promotion Prevention and Response Branch \nCenters for Disease Contol and Prevention \nRonda Cochran\, MPH    \nDeputy Team Lead Hospital Infection Prevention Team \nPrevention and Response Branch Division of Healthcare Quality Promotion (DHQP) \nCenters for Disease Control and Prevention \nRachel Snyder\, MPH  \nScientific Data Analyst Chenega Enterprise Systems and Solutions (chESS) \nPrevention and Response Branch Division of Healthcare Quality Promotion (DHQP) \nCenters for Disease Control and Prevention \n  \nWatch the Recording\nView the Slides

SUMMARY:IPRO HQIC Sepsis Affinity Group: Mid-to-large sized Hospitals
DESCRIPTION:Sepsis remains a national priority and is a complex and challenging condition to manage. Despite substantial attention to improvement\, an opportunity still exists to decrease sepsis-related morbidity and mortality. More than 1.7 million adults in the U.S develop sepsis every year\, leading to over 270\,000 deaths (CDC\, 2020). \nHospitals are committed to improving care\, outcomes\, and costs; however\, many have expressed they wrestle with where and how to start their quality improvement efforts. \nIPRO is launching a 6-month Sepsis Affinity Group to help support and provide a networking forum specifically for mid-to-large sized hospitals as they work to reduce sepsis-related harm. With the shared goal of accelerating learning and improvement\, participants will have access to content experts and a platform to share experiences\, evidence-based practices\, and tools\, as well as explore successes\, challenges and lessons learned to drive progress on sepsis-related harm. Some topics to be included are sepsis screening\, SEP-1 bundle compliance and reporting\, and more. \n  \nOn behalf of the IPRO HQIC team\, we look forward to your joining and working together with the Sepsis Affinity Group! \n  \nView Slides\n
