SUMMARY:Partnering Beyond COVID-19: Learning Circle: Heart Health
DESCRIPTION:Session Topic: Learning Circle: Heart Health \nDescription: Review and Check-in   \nCOVID-19 has changed healthcare in many ways and has delayed chronic disease management for many patients. Patients needing follow-up appointments for their chronic disease were not able to schedule appointments due to decrease in in-person visits\, laboratory closures for routine testing\, and less access to community-based programs.  \n  \nOur program over the next few months will share evidence based practices and innovative strategies that have emerged to improve prevention and management of chronic disease and outcomes.  \n  \nUpon completion of this educational series\, participants will be able to:  \n\nParticipants will develop deeper understanding of best practices and evidence-based tools for prevention and management of hypertension\, chronic kidney disease (CKD)\, and diabetes\, \nParticipants will enhance their knowledge of Million Hearts\, TAKEHeart and IPRO QIN-QIO tools for increasing referrals to and participation in cardiac rehabilitation programs\, and \nParticipants will enhance their knowledge about chronic disease health disparities and the best practices for addressing these disparities\, to support better health outcomes. \n\n  \nAudience:  Partnership for Community Health\, hospitals\, skilled nursing facilities\, home health agencies\, physician practices\, and community-based organizations.

SUMMARY:CLAS Theme 3\, CLAS Standard 9\, and CLAS Standard 10
DESCRIPTION:  \nIn this final session of the CLAS series\, IPRO will share additional resources that participants might find helpful to support CLAS:  \n\nCMS Office of Minority Health Resources \nAHRQ Resources \nIPRO Resources \n\nFinal Q&A  \nWrap-Up  \n\n  \n\nWhat is CLAS?\nCulturally and linguistically appropriate services (CLAS) are a way to improve the quality of services provided to all individuals\, which will ultimately help reduce health disparities and achieve health equity. CLAS is about respect and responsiveness: Respect the whole individual and Respond to their health needs and preferences. \nWhy is CLAS Important?\nCLAS helps advance health equity to ensure that every person can “attain his or her full health potential” and no one is “disadvantaged from achieving this potential because of social position or other socially determined circumstances.” While many factors impact health equity\, providing culturally and linguistically appropriate services is an effective strategy to ensure better health outcomes for diverse populations. \nJoin IPRO for a webinar series covering the themes and standards based on the HHS National Standards for Culturally and Linguistically Appropriate Services in Health and Health Care.  Complete the IPRO CLAS Assessment to understand where your organization is with its implementation of CLAS and use that to develop an action plan to address opportunities for improvement. This webinar series will walk through how to develop an action plan\, best practices\, and resources to help implement your action plan.
