DESCRIPTION:(QSO-23-16-Hospitals Memorandum) \nComprehensive and effective discharge planning is imperative to ensure continuity of care to support improved patient/resident health outcomes and safety at transfer and discharge to the community. This webinar program will provide an overview of the 6/6/23 Centers for Medicare & Medicaid Services (CMS) Quality\, Safety & Oversight (QSO) 23-16 – Hospital Memorandum of Requirements for Hospital Discharges to Post-Acute Care Providers (https://www.cms.gov/files/document/qso-23-16-hospitals.pdf) and focus on areas identified by CMS as opportunities for improvement related to inclusion and accuracy of critical patient information.
