DESCRIPTION:  \nIn this session\, IPRO and participants will discuss: \nCLAS Standard 9:  \n\nEstablish culturally and linguistically appropriate goals\, policies\, and management accountability\, and infuse them throughout the organization’s planning and operations. \n\nCLAS Standard 10:  \n\nConduct ongoing assessments of the organization’s CLAS-related activities and integrate CLAS-related measures into measurement and continuous quality improvement activities. \n\nCLAS Standard 11:  \n\nCollect and maintain accurate and reliable demographic data to monitor and evaluate the impact of CLAS on health equity and outcomes\, and to inform service delivery \n\n\n  \n\nWhat is CLAS?\nCulturally and linguistically appropriate services (CLAS) are a way to improve the quality of services provided to all individuals\, which will ultimately help reduce health disparities and achieve health equity. DESCRIPTION:Session Topic: Learning Circle: Heart Health \nDescription: Review and Check-in   \nCOVID-19 has changed healthcare in many ways and has delayed chronic disease management for many patients. Patients needing follow-up appointments for their chronic disease were not able to schedule appointments due to decrease in in-person visits\, laboratory closures for routine testing\, and less access to community-based programs.  \n  \nOur program over the next few months will share evidence based practices and innovative strategies that have emerged to improve prevention and management of chronic disease and outcomes.  \n  \nUpon completion of this educational series\, participants will be able to:  \n\nParticipants will develop deeper understanding of best practices and evidence-based tools for prevention and management of hypertension\, chronic kidney disease (CKD)\, and diabetes\, \nParticipants will enhance their knowledge of Million Hearts\, TAKEHeart and IPRO QIN-QIO tools for increasing referrals to and participation in cardiac rehabilitation programs\, and \nParticipants will enhance their knowledge about chronic disease health disparities and the best practices for addressing these disparities\, to support better health outcomes. \n\n  \nAudience:  Partnership for Community Health\, hospitals\, skilled nursing facilities\, home health agencies\, physician practices\, and community-based organizations.
