DESCRIPTION:Wednesday\, October 9 from 3 to 4 pm ET\nRegistration Link: https://bit.ly/47jRmnm\nLearning Objectives\nIn this webinar\, we will: \n\nDefine the characteristics and benefits of a Learning Health System\nDescribe foundational POLLC activities to support Learning Health Systems aimed at advancing evidence driven pain and opioid management and improving patient outcomes\nProvide a Round Table on lessons learned from POLLC participants\nEnable participants to design their LHS leveraging POLLC tools and resources\n\nAudience\nThis activity is appropriate for all those interested in improving pain and opioid management and outcomes\, and/or improving Learning Health Systems for other conditions. This includes: \n\nClinicians (e.g.\, Physicians\, NPs\, PAs\, Nurses\, Pharmacists)\, Public Health Professionals\, Patients and/or Care Partners\nHealth Systems\, Hospitals\, Primary Care\, Specialty Clinics\, Nursing Homes\, and Health IT vendors\n\nSpeakers\nJerry Osheroff\, MD – Principal\, TMIT Consulting\, LLC\nDr. Jerry Osheroff is a primary care physician by training who has focused for over 30 years on developing and implementing approaches to improve healthcare delivery processes and outcomes. He is a principal of TMIT Consulting\, LLC and a care transformation subject matter expert helping VA become a learning health system and achieve the quintuple aim. He is the lead author on influential guidebooks and national roadmaps for achieving these goals and brings together diverse stakeholders to implement the recommendations (e.g.\, via initiatives such as POLLC and ACT) and accelerate improvements in care delivery and results. \nAnne Myrka\, RPh\, MAT\, CPHQ – Senior Director of Drug Safety ad Chronic Disease Management\, IPRO\nAnne Myrka earned a Bachelor of Science in Pharmacy from Albany College of Pharmacy and Health Sciences and a Master of Arts in Teaching Science from the State University at New Paltz. She is a licensed pharmacist in New York and Vermont. She has presented nationally about high-risk medication management\, care transitions\, and medication reconciliation\, and has co-authored multiple peer-reviewed journal articles on these topics. \nLynn Wilson\, MS – Senior Quality Improvement Specialist\, IPRO\nLynn Wilson has more than 30 years of experience with regulatory agencies for community mental health centers and nursing homes and is a nationally recognized QI leader in long-term care\, behavioral health and hospice and palliative care settings. Under her leadership\, behavioral health and end of-life care practice innovations generated through frontline staff quality improvement processes have been recognized as national best practices.
